Request for personalised application form (Initial Application Form) for registration with An Bord Altranais. This form should be completed by nurses/midwives applying to register from European Union and Non-European Union countries.

This form must be completed if you have successfully completed an education programme for Prescriptive Authority in Ireland and you already have your name entered in the Register of Nurses and have a Personal Identification Number.

This form must accompany the Application Form for Registration in the Registered Nurse Prescribers Division if you completed an education programme for Prescriptive Authority in Ireland and you already have your name entered in the Register of Nurses and have a Personal Identification Number.

This form must be completed if you have commenced a programme of nursing studies in Ireland and you already have your name entered in the Register of Nurses and have a Personal Identification Number.

This form must be completed if you wish to obtain an An Bord Altranais Nursing Badge.

This form must be completed if your name has been removed from the Register of Nurses for Non-payment of Fees.

This form must be completed if you have lost or misplaced your Annual Certificate or Initial Certificate.

This form must be completed if your Annual Retention Certificate was issued by An Bord Altranais but you have not received same.
